Form DS-6004 for Guidance in Preparing the Submission of a “Request to Change End User, End Use, and/or Destination of Hardware”
DDTC plans to receive and adjudicate Requests to Change End User, End Use, and/or Destination of Hardware electronically via the D-Trade system. A form, DS-6004, has been designed for this purpose and is being developed for D-Trade use. In the interim, DDTC offers this form and instructions as guidance for the preparation of such requests. This form is not meant for hardcopy use. Please do not fill out the form and send it to DDTC for review (it will be Returned Without Action).
Once the DS-6004 is fully developed for the D-Trade system, its use will be mandatory. Adherence to the guidelines as captured in this form and accompanying instructions even at this time, however, can help ensure that your Request to Change End User, End Use, and/or Destination of Hardware has all the information that is necessary and sufficient for timely, accurate U.S. Government review.
A further announcement will be made when Form DS-6004 is integrated into D-Trade and its use becomes mandatory.
