A Complete Registration Package

A complete registration package consists of the following:

1. DS-2032 Statement of Registration



2. U.S. or foreign equivalent Government or State (endorsed or issued) documentation supporting the legal name selected in Block 4 of the DS-2032 and organizational type of the registrant selected in Block 6 of the DS-2032. Foreign registrants must submit an official English language translation for documentation in other languages. Examples of accepted documentation are:




a) State Incorporation Certificate, or



b) State Certificate of Good Standing, or



c) State Articles of Incorporation, sole proprietorship, partnerships, limited liability partnership/company,
    and corporations



3. Applicable Registration Fee
