Changes to Ultimate Parent or Subsidiaries/Affiliates (Involved in ITAR-Related Activity) (Per ITAR §122.4(a)(2))

Changes to previously submitted information regarding the registrant's ultimate parent, intermediate owners, subsidiaries, or affiliates involved in ITAR-related activity must be submitted within 5 days of the effective date. If the change is due to a merger or acquisition, refer to the merger/acquisition guidelines.

Provide the following information for each firm:

• Effective date of change and an explanation for the revision (e.g., newly formed entity)
• Name of ultimate parent, intermediate owners, Subsidiary, or Affiliate
• Physical street address
• City
• State/Province
• Country
• Zip/Postal Code
• Telephone number

DDTC will only send an acknowledgement letter regarding ultimate parent or subsidiary/affiliate changes, when Industry informs that licenses, agreements and / or other authorizations are to be transferred. Otherwise retain the signed registered or overnight mail receipt as proof of submission.
